St Anthony’s Transition to School Program
This program aims to support the child to enable smooth transition between the early childhood setting and Kindergarten. The program will offer support for the child through appropriate assessment and curricular planning; liaising with the family through adequate preparation and their active involvement; and support for the next educational environment through appropriate planning and ongoing communication.
Transition to kindergarten brings a significant change in curriculum and in the amount of support that can be provided to the child with additional needs. Transition to a new program creates challenges for the child, family, teachers and other service providers. Transition planning assists children and their families as they move into a new relationship with service providers. For transitions to be successful the process must offer support for a child through appropriate assessment and curricular planning; support for the family through adequate preparation and their active involvement; and support for other service providers through appropriate planning and ongoing communication. Planning can help a child and family reduce anxiety about transition and deliver a positive experience for the child. Communication among educators and other service providers and parents will ensure that specific child needs and expectations are identified and addressed and potential problems avoided.
Please note, this program can only take place with the consent from the child’s current childcare centre and proposed school.

Duration: This program should commence 6 months prior to the child commencing school. Up to 5 school supported visits. Approximately 2-3hr session once per week are recommended.
Cost: $ 150 per visit
Participation in Learning Support Team Meeting: $ 150 / meeting
